Find out how we can help you by scheduling an “options assessment” with our Nursing Manager. This assessment is usually conducted with the patient and family in your home, in our office or in another comfortable location of your choice. We encourage all involved family members to attend so that we listen to the whole story, enabling all of the important information to be highlighted. After a complete review, we customize a care plan that includes options on how to proceed on your own or by engaging Qualicare.

This options assessment is an opportunity to ask questions, receive guidance and begin making decisions on how to move towards appropriate goals, which are established and agreed to by patient and family.

• We assess client needs and family circumstances to create care plans with specific goals. Our case management service provides on-going assessments and consistent care. Care plans are modified as requirements change, demonstrating our priority of providing exceptional care, while maximizing quality of life.
• Clients are assigned a Personal Care Manager who is a Registered Nurse with both medical and social experience, which enables them to assist both physically and emotionally.
• We consider the whole picture in setting goals. With solution-focused healthcare, clients receive special attention, based on their unique circumstances.
• Reducing chaos to avoid medical crisis and stay out of hospital, is a key objective of Qualicare. We coordinate both public and private resources providing clients with consistency of care. No matter what skills or resources are needed, we will offer or access them to provide a full continuum of care.

You may choose care on-demand, as needed or one of our Service Packages. Clients who choose a package receive around-the-clock on-call service. This includes access to an assigned Registered Nurse Care Manager, who is available to the patients, caregivers and family members. This approach helps keep our patients out of the hospital and helps family members have peace-of-mind.

We care for patients in private homes, retirement communities, nursing homes and hospitals. When you choose Qualicare, we will review the medical history, determine current status and discuss critical issues that require immediate focus. We collect information on medicines, doctors, programs and institutions involved in care to be used during doctor or hospital visits. We work with families to establish appropriate priorities for care with suggestions on follow-up activities. Most importantly we will stay involved so that you are not left alone and can depend on getting consistent coordinated care.
