An ideal time to call us is before a hospital discharge. Our participation in the discharge planning can help ensure that patients receive the optimal government services, properly complemented with private services. In fact, if possible, patients should call before being admitted to the hospital to ensure they are well prepared.

Our early involvement often accelerates a patient’s hospital departure. It also enables us to ensure that proper homecare is in place, necessary supplies and equipment are available, bathroom and bedroom modifications are prepared in time and that the transfer home from the hospital is smooth, successful and safe. Throughout this process, the patient’s comfort and dignity and the family’s peace of mind always remains our top priority.

When in hospital; extended medical insurance, which is available in employee benefit plans, will often pay for care by Personal Support Workers (PSW's) or Registered Nurses, when required. Check your benefit packages for reimbursements.
