Our Personal Care Managers are Registered Nurses, with years of experience, earning them the ability of providing crisis intervention and healthcare support. With a Qualicare PCM (or Nurse Manager) on your team, you always have someone to turn to who is compassionate and professional. With our PCM you can count on individual attention and consistent care. Your PCM can assume a role as a 'single point-of-contact' to coordinate health care activities and manage your professional healthcare team.

Our priority is to provide excellent care with exceptional people. Qualicare Nurse Case Managers and caregivers are properly educated, caring people. We use a comprehensive evaluation and satisfaction measurement process for the caregivers on your team. Our Nurse Case Managers (PCMs) set care plans for our PSW's (caregivers) and then manage and monitor them against those plans. In fact, with the close contact and support we offer our caregivers: our homecare is second to none.

A Nurse Case Manager considers all factors to understand the entire picture, in order to set realistic and appropriate goals. Our special care is solution focused and based on each patient’s unique circumstances. We will help save time and valuable energy by handling issues that arise. Case Management

When illness strikes, your family is suddenly faced with an overwhelming new reality. As aging becomes challenging, old tasks seem daunting. There are physical issues to deal with and emotional ones. Registered Nurse Case Managers (PCM's) are assigned to clients who want privately, case-managed care. These Nurse Case Managers are able to be involved with patients and their families to coordinate their healthcare. The guidance and support they provide reduces the burden on families, allowing family members to stay involved in a more meaningful and helpful way. Our Nurse Care Managers (PCMs) collaborate with the client's physicians, hospitals and social agencies to leverage all resources available, including both public and private services, enhancing the level of care and quality of life. They follow a doctor’s lead, utilizing our full range of services, described in our Services section.

All Service Packages we offer include case management services. All packages include an on-call nurse manager (PCM) 24/7, for safety, security and support. Clients with case managed care are also able to acquire medical equipment and supplies conveniently and at reduced prices.
