Around-the-Clock Care

Qualicare worked with the hospital to arrange Sarah's discharge, provided around-the-clock nursing, home support services and organized Sarah's transfer to her home. Sarah's Personal Care Manager became the single point of contact for all her healthcare requirements. As Sarah's condition stabilized and community home care became available, Qualicare coordinated the public services she was entitled to and supplemented her care as required. In addition, Sarah's PCM remained involved to monitor her health and to manage her healthcare team and services.
